Sheade Therapy and Consulting ("we," "us," or "our") is a d/b/a of Ryan M. Sheade LCSW LLC, an Arizona limited liability company. This Privacy Policy explains what information we collect through www.SheadeTherapy.com (the "Site"), how we use it, and the choices you have.

This policy covers information collected through this website. It does not govern protected health information created once you become a client. That information is governed by our Notice of Privacy Practices, which you receive at intake and may request at any time by emailing support@integratedmha.com.

1. Do Not Send Clinical Information Through This Website

Email and website contact forms are not secure and are not HIPAA-compliant channels. Please do not use them to share diagnoses, symptoms, treatment history, medication details, or other sensitive health information.

To request services, tell us only your name, preferred contact method, and general availability. Once you are a client, all clinical communication moves to our secure client portal.

This website is not monitored for emergencies. If you are in crisis, call or text 988 (Suicide & Crisis Lifeline), call 911, or go to your nearest emergency department. In Maricopa County, you may also reach the Solari Crisis Line at 1-844-534-4673.

2. Information We Collect

Information you give us directly:

Name, email address, phone number, and preferred contact method
General reason for reaching out and requested appointment availability
Insurance carrier name, if you tell us
Registration details for consultations, groups, or trainings
Email list signup information
Any other content you choose to type into a form or message

Information collected automatically:

IP address, browser type, device type, operating system
Pages viewed, time on page, referring website, links clicked
Cookies and similar technologies placed by our website host and analytics tools

Our Site is hosted on Wix, which collects certain technical data as our service provider.

Payment information: We do not collect or store payment card numbers on this Site. Payments are processed by wix, which maintains its own privacy and security terms.

3. How We Use Your Information

We use the information above to:

Respond to your inquiry and determine whether we are a clinical fit
Schedule consultations, appointments, groups, and trainings
Verify insurance eligibility and benefits
Send appointment reminders and administrative messages
Send educational or announcement emails, if you opted in

We do not sell your personal information. We do not rent, trade, or share it for third-party advertising.

4. Who We Share Information With

We share information only with:

Service providers who help us operate — website host, scheduling and records platform, secure email provider, payment processor, billing service. Each is bound by contract, and those handling protected health information sign a Business Associate Agreement.
Health plans, when you have asked us to bill your insurance.
Others as required or permitted by law, including mandatory reporting of suspected abuse or neglect of a child or vulnerable adult, response to a valid court order or subpoena, and duty-to-protect situations under Arizona law.

5. Cookies and Your Choices

You can set your browser to refuse cookies or alert you when cookies are being sent. Some parts of the Site may not function properly if you disable them. Where required, our cookie banner lets you accept or decline non-essential cookies.

To stop receiving marketing email, click "unsubscribe" in any message or email support@sheadetherapy.com. You cannot opt out of transactional messages (appointment confirmations, billing notices) while you are an active client.

If you consent to text messages, standard message and data rates apply. Reply STOP to opt out or HELP for assistance.

6. Children's Privacy

This Site is intended for adults. We do not knowingly collect personal information from anyone under 13. Parents and legal guardians submit information on behalf of minor clients. If you believe a child under 13 has submitted information through this Site, email us and we will delete it.

7. How Long We Keep Information

Website inquiries that do not result in services: deleted within 24 months.
Email marketing lists: kept until you unsubscribe.
Clinical records: retained as required by Arizona law (A.R.S. § 12-2297) — at least six years after the last date of service for adult clients; for clients who were minors at the time of service, until three years after the client's 18th birthday or six years after the last date of service, whichever is later.

8. Security

We use administrative, technical, and physical safeguards appropriate to the sensitivity of the information we hold, including encrypted storage, access controls, and a HIPAA-compliant records platform. No method of transmission over the internet is completely secure, and we cannot guarantee absolute security.

9. Your Rights

You may request that we access, correct, or delete information you submitted through this Site by emailing support@sheadetherapy.com. We will respond within 30 days.

Requests regarding your clinical record are handled under HIPAA and Arizona law, not this policy. Submit those in writing, and we will respond within the timeframes those laws require.

10. Changes to This Policy

We may update this Policy. The "Last Updated" date above reflects the most recent version. Material changes will be posted on this page.
